A breast reduction is performed if the breasts are too large, also known as mammary hyperplasia or macromastia. In most cases, a chest that is too heavy causes severe back and neck pain, which can lead to postural problems in the long term. We speak of gigantomastia when the breast tissue weighs over 1,500g per side. The average weight of a woman who is not breastfeeding is 150-400g. There may also be large asymmetries in breast size and weight. Breast reduction, possibly with a symmetry adjustment, can provide a remedy here.
Common reasons for breast surgery
A breast reduction can be a real relief for women who suffer from the physical discomfort of excessively large breasts. By reducing the volume and weight of the chest, back and neck pain are often significantly reduced, resulting in improved posture. In addition to the health benefits, breast reduction also offers aesthetic corrections by balancing asymmetrical breasts and harmonizing the appearance of the breast.
Dr. Berkei uses the so-called T-incision for breast reduction, a precise technique to achieve an aesthetically pleasing breast reduction. The incision runs around the areola and leads vertically downwards, ending in a horizontal line below the inframammary fold. Through these incisions, Dr. Berkei removes excess fat and glandular tissue as well as skin. She then shapes the remaining breast into a well-formed contour. The nipple is moved upwards and the areola is often reduced in size. Although this method leaves scars in the shape of an inverted T, the scars in the nipple area are barely visible and the horizontal part of the T lies discreetly in the underbust crease.

The loss of skin elasticity due to the natural ageing process, genetic predisposition, pregnancy or major weight fluctuations can cause the skin on the breasts to sag. Even regular exercise often cannot counteract this process, as the shape of the breast depends mainly on the condition of the breast tissue and the surrounding skin. In such cases, a breast lift can be an effective solution to tighten the breasts and give them a more youthful shape. In contrast to a breast reduction, a breast lift aims to maintain the existing breast size and only remove the sagging skin and reshape the breast tissue. While a breast reduction involves removing excess breast tissue, a breast lift focuses mainly on tightening the skin and shaping the breast to achieve an aesthetically pleasing result.
The cutting technique
A tried and tested method for breast reduction is the T-shaped technique, which allows the sagging tissue to be repositioned. This method restructures the internal tissue and uses it as a natural replacement for an implant. The result: a lasting increase in volume and a fuller décolleté with contours that retain their shape. The I-shaped technique is a less scarring option. The incision is made from the nipple downwards, without a horizontal incision in the underbust crease.
Breast lift with breast augmentation
Many women are not only looking to lift their breasts, but also to enlarge them and give them an attractive shape. Tightening alone often leads to a loss of volume. A combination operation is a good option here, in which both the tightening and the insertion of an implant or the use of autologous fat can be performed in one session. This combined method makes it possible to achieve various aesthetic goals in one procedure, which is both time and cost efficient.
Breast lift with liposuction
Some women have a breast shape that is more elongated than round, as the breast tissue extends far beyond the side of the rib cage. In some cases, there are also small fat pads in the armpit region. In order to achieve an aesthetically pleasing, round breast shape, liposuction can also be performed in the lateral chest or armpit area in addition to a breast lift alone. This additional measure helps to optimize the contours of the breast and create a harmonious overall appearance.

For your safety and the best possible healing process, Dr. Berkei has defined special, individually tailored aftercare steps as part of the B Safe package. Optimized and perfected through the knowledge gained from countless breast operations and thousands of patients treated, B Safe enables you to heal particularly quickly, safely and painlessly. Before the procedure, we will give you detailed instructions and recommendations that you can follow for optimal healing.
We recommend that you wear a support bra for at least 4 weeks after the operation. This bra is put on you before the operation and is specially designed to support and stabilize the breasts during recovery without putting strain on the sensitive surgical site. The support bra can be washed at 30 degrees if necessary and is also suitable for tumble drying.
During the surgical procedure, Dr. Berkei carefully inserts drainage tubes into the breast. These drains are extremely fine tubes that are used to drain wound fluid and thus support healing. They enable a controlled drainage of fluid from the operating area and guide it safely into a container where it is collected. These drains usually remain in place for about 3 days to a week after breast surgery and are then removed painlessly. Once the drains and the container have been removed, you can shower again the next day.
In the first two weeks after the operation, it is essential to rest and avoid any physically strenuous activities such as sport, housework or gardening. We recommend that you spend the first week in bed and the second week on the sofa to provide optimum support for wound healing. Also avoid excessive lifting of the arms during this time, as this can lead to tension at the edges of the wound, which could impair the healing process. In addition, you should not drive yourself for the first two weeks.
From the third week onwards, you can slowly return to your daily routine. Please note, however, that sporting activities, sunbathing and sauna visits should only be resumed after 6-8 weeks and in consultation with our medical team. These steps are crucial to ensure a smooth recovery and achieve the best possible outcome.
Scars leave their mark after every operation, which is why regular care is essential. Due to the weakened skin and connective tissue, minor wound healing disorders can occur, particularly at the T-spots. For this reason, visits to the solarium and sunbathing should be avoided in the first few months after the breast lift. In order not to impair wound healing, it is advisable to abstain from cigarettes and other nicotine-containing substances for 6 weeks before and after the operation. Irritation caused by bras that are too tight should also be avoided. We will be happy to prescribe suitable care products with silicone during the pre-op consultation.
In the first year after the operation, daily massages can help the scars to heal. If necessary, we will show you how to do this during a check-up. We can also recommend a lymphatic drainage and connective tissue massage on request. Manual lymphatic drainage is performed by qualified physiotherapists to support your recovery in the best possible way.

The nipples often sink to a level below the underbust crease, which we refer to as nipple ptosis. As part of a breast lift or reduction, the nipples are lifted and repositioned.
We operate on adult patients from the age of 18.
Unfortunately, scars are often unavoidable during surgery. With breast augmentation, the scar is hidden in the underbust fold and is virtually invisible. In the case of a breast lift or reduction, there may be additional scars around the areola and a longitudinal scar. All patients are instructed postoperatively on how to care for their scars and are given silicone products. We also offer laser treatment of scars for our patients. Approximately 6-12 months after the operation, we recommend special scar tattoo artists who pigment skin-colored pigments into the scars with beautiful results.
We recommend that our patients take a rest period of 2-3 weeks. However, by the third week you are already socially acceptable again. Sport and lifting heavy objects should be avoided for approx. 6 weeks.
If you are satisfied with the volume of your breasts, we usually recommend breast lift surgery (so-called masopexy). If the skin of the breast is sagging and the volume is too large, we recommend a breast reduction. We will be happy to advise you in a personal consultation as to which of the procedures is right for you.
As we are a private medical practice, we cannot bill the statutory health insurance. In the case of private insurance, however, parts of the operation fee can be covered by the health insurance if it is a medical indication. We will be happy to advise you on this in a personal meeting.
During a breast lift, excess skin is removed and the breast, including the nipple, is lifted to its original position. An attempt is made to maintain the breast size or to increase it using autologous fat or implants.
As part of a breast reduction, additional breast tissue is removed to reduce the size of the breast. This is particularly helpful for patients who suffer from excessively large and heavy breasts and experience shoulder and neck pain as a result.
The surgical fee depends on the individual findings and the extent of the operation. We will be happy to advise you on this in a personal meeting. At the end of the consultation, you will find out exactly what costs you can expect.
On the day of the operation, Dr. Berkei will talk to you again and explain the rest of the procedure. You are welcome to bring us any pictures you wish, which we will take with us to the operating theater. Once you have been signed in, our anesthesia team will talk to you before you are taken to the operating room. You will be prepared there.
The operation is performed under intravenous general anesthesia; nausea or vomiting are not to be expected as this is a particularly gentle anesthetic procedure.
After the operation, you will remain in the recovery room for a short time, where Dr. Berkei will visit you and explain the surgical procedure. You will then be taken back to the ward. You are welcome to receive visitors there and will be provided with everything you need by our ward nurses. You stay at the clinic for 2 nights and are visited daily by our team of doctors. You will then be discharged home and return to our practice for further check-ups.
A breast lift can provide long-term results, but it is not necessarily a permanent procedure. The results can be influenced by various factors such as ageing, weight fluctuations, hormonal changes and gravity. These factors can cause the breast shape and firmness to change again over time. It is also possible that additional interventions may be necessary in the future to maintain the desired appearance.
