Advantages of breast implants
Breast augmentation can be performed using different techniques and implants can be inserted into the breast using different incisions: The periareolar incision around the nipple, the inframammary incision under the breast crease and the transaxillary incision in the armpit. The most frequently performed and most sensible method is the inframammary incision. The implant is inserted via the underbust fold. The incision, which depends on the size of the implant and is about 4 cm long, allows us a good field of vision and precise positioning of the implant, regardless of its size. The incision is made in such a way that it is later placed as directly as possible in the underbust crease and thus hidden. Once healed, the scar on the underbust crease is usually light and fine, making the procedure barely visible and the results appear particularly natural.

The choice between silicone implants and autologous fat tissue transplantation is often a decisive one for women who want breast augmentation. While breast augmentation with silicone implants is the most commonly chosen method, autologous fat grafting is becoming increasingly popular. Around a third of all breast augmentations in Germany are already performed using the patient’s own fatty tissue, and this trend is on the rise.
Autologous fat tissue transplantation offers a perfect alternative to classic breast augmentation with silicone implants. The body’s own fatty tissue is used to enlarge the breast without the need for implants. This procedure has a high healing rate of 60 to 80% of the transplanted fat cells, which depends primarily on the surgical technique but also on the personal take rate. Dr. Berkei places particular emphasis on gentle removal of the fatty tissue, careful processing using special technology and precise transplantation of the patient’s own fat in the form of 3D microinjections into the breast. Minimal stitches and the finest cannulas result in only tiny, almost invisible scars of 1-3 millimeters in size.
Advantages of autologous fat transplantation
Breast augmentation with autologous fat is particularly suitable for moderate breast augmentation, asymmetrical breasts or tubular breasts. It can also be performed as a hybrid operation, in which both implants and the patient’s own fatty tissue are used, particularly to build up the décolleté. This is particularly relevant for slim patients in order to conceal possible marks from implant edges.
The special thing about the breast after an autologous fat transplant is that it feels exactly like the tissue of a natural breast. Once healed, fat cells remain in place and behave like normal fatty tissue. There are no defense reactions of the body, as can occur with implants in the form of capsular fibrosis.
Breast augmentation with autologous fat offers a win-win situation all along the line. In this procedure, annoying fat deposits in problem areas such as the abdomen, hips or legs are suctioned out and the fat tissue obtained, our “liquid gold”, is used for breast augmentation. A complete body shaping is carried out in just one operation, so to speak. About half a cup size of volume can be transplanted during an operation. If there is enough of the patient’s own fat tissue from the body areas in question, the operation can be repeated after 3-6 months to achieve a further increase in volume.

Hybrid breast augmentation, also known as composite breast augmentation, combines the best of two worlds in breast surgery. Customized implants are used and the patient’s own fat is gently injected. This method makes it possible to conceal the edges of the implants with the patient’s own fat in order to achieve a natural result while minimizing the visibility of the implant edges. This effectively treats the problem of implant visibility in slim women, while the goal is always an aesthetic and natural appearance.

Hybrid breast augmentation offers a number of additional advantages over conventional procedures. By combining implants and autologous fat, the size, shape and contour of the breast can be individually adapted to your needs and wishes. Modern breast surgery now offers an extensive selection of breast implants in different sizes and shapes. Dr. Berkei and her colleagues will guide you through the variety and help you find the perfect implant.
Implants are available in various materials, including silicone cohesive gel, bi-light implants with incorporated air bubbles, sterile saline solutions and two-chamber cushions. With different surface structures, smooth or textured, they offer advantages such as a lower likelihood of complications such as capsular fibrosis or dislocation. The implant shapes range from round to anatomical or teardrop-shaped variants, while the diameter and projection can vary from flat to extra high. Among the leading manufacturers that meet the strictest safety and quality standards are names such as Polytech Health & Aesthetics, Allergan/Natrelle, Eurosilikone, Motiva, Silimed and Mentor.
The implant shape
There are two main types of breast implants: round and anatomical (also known as teardrop-shaped) implants. Round implants have an even curvature and ensure a plump décolleté with round breasts that appear fuller in both the upper and lower areas.
Anatomical implants are modeled on the natural shape of the female breast and give the breast natural contours, especially in the lower area. In addition, there are further subcategories of anatomical implants, depending on the shape of the base plate, including longitudinal oval, round-based and transverse oval implants.
The choice of implant shape often depends on individual anatomical features such as the width of the breasts and the length of the underbust area.
The implant thickness
In addition to the implant shape, both the size and the so-called projection (implant thickness) are decisive for the selection of an optimal implant that fits the woman’s body. There are essentially four different projections of an implant: low, moderate, high, extra-high. The volume of breast implants ranges from 80 to 600 grams. In our clientele, women generally request an implant size between 220 and 430 grams, in the USA it is 320 to 530 grams.
The implant surface
Breast implants are made of various biocompatible materials, although most implants used in Germany are filled with high-quality silicone gel. The shell of an implant is usually also made of silicone. The surface of the implant can be either smooth or textured. Textured surfaces promote better adhesion to the surrounding tissue, which reduces the risk of implant displacement or rotation and reduces the occurrence of painful hardening around the implant (capsular fibrosis). Implants with a smooth surface, on the other hand, are free to move, while implants with a rough surface can no longer move in the breast pocket over time. Polyurethane foam implants offer similar advantages to rough textured implants and are preferred by Dr. Berkei due to their anti-rotation properties.
Worldwide selection of implant brands
In Dr. Berkei’s practice, we prefer to use polyurethane (PU) coated implants from Polytech, which offer numerous advantages. These implants have proven to be extremely safe, with less than 1% of cases of BIA-ALCL (breast implant associated large anaplastic lymphoma ) related to Polytech implants. The risk of capsular fibrosis within 10 years with these implants is only 1-3%, which is very low compared to other implant types. They also have the lowest risk of rotation, capsular fibrosis, post-operative bleeding and infection. We will advise you and pass on our many years of experience on the advantages and disadvantages of other breast implant providers such as Motiva, Mentor, Allergan/Natrelle, Eurosilicone and McGhan.
The implant position
The choice of the correct positioning of the breast implant is largely dependent on individual factors such as the existing tissue, body structure and aesthetic goals. Each technology offers specific advantages and disadvantages that should be carefully weighed up in the context of individual needs.
In the subglandular or epimuscular technique, the implant is placed on the pectoral muscle, which is usually only used in women with sufficient own tissue and feminine curves. As a result, the implant is well concealed and is neither visible nor palpable. However, this method could lead to an unsatisfactory result in women with too little own mass and the desire for larger implants, as the implant would probably be visible and palpable. One advantage of this technique is that the implant can be placed without affecting the pectoral muscle, which is less traumatic for the tissue overall and reduces the risk of post-operative bleeding.
With the submuscular method, however, the breast implants are located partially or completely under the pectoral muscle. This position is particularly suitable for very slim women with little or no fat and glandular tissue of their own, as the implant is covered by the breast muscle and the edges are not visible. However, this method is less suitable for women with pronounced chest muscles, as the implants are under tension and may become deformed.

In general, our patients are almost pain-free after just a few days. Most only report tension and back pain. However, we recommend allowing a grace period of 2-3 weeks. If you would also like to support the healing process, we will be happy to advise you on nutritional supplements or infusions that have a positive effect on wound healing.
Yes, after breast surgery it is usually necessary for you to stay in hospital for about two nights for monitoring and safety reasons. This helps to identify and treat any complications at an early stage and to ensure that your recovery is optimal.
Most of the post-operative swelling has already subsided after 3-4 weeks. However, the final result is only visible after approx. 3-6 months, when the skin has stretched.
On the day of the operation, Dr. Berkei will talk to you again and explain the rest of the procedure. You are welcome to bring us any pictures you wish, which we will take with us to the operating theater. Once you have been signed in, our anesthesia team will talk to you before you are taken to the operating room. You will be prepared there.
The operation is performed under intravenous general anesthesia; nausea or vomiting are not to be expected as this is a particularly gentle anesthetic procedure.
After the operation, you will remain in the recovery room for a short time, where Dr. Berkei will visit you and explain the surgical procedure. You will then be taken back to the ward. You are welcome to receive visitors there and will be provided with everything you need by our ward nurses. You stay at the clinic for 2 nights and are visited daily by our team of doctors. You will then be discharged home and return to our practice for further check-ups.

We generally recommend having a breast ultrasound scan once a year.
Specialized radiology practices can also perform a mammogram with a horizontal breast implant. However, you should inform the radiologist of this when making the appointment.
If a mammography is not possible in the respective facility, an MRI examination of the breast can be carried out in any case.
The size of the implant depends not only on the patient’s wishes, but also on her anatomy. We will be happy to advise you on this in a personal meeting. As part of our consultation, we will work with you to determine the appropriate implant shape and size.
We only use breast implants from Polytech – this is the only manufacturer of breast implants that produces in Germany.
From Polytech’s product portfolio, we generally use implants with a polyurethane coating, as these cannot slip or rotate and statistically have a very long acceptance period by the body. Whether a round or trough-shaped implant is used depends on the individual findings and the wishes and expectations of our patient.
We recommend that our patients take a rest period of 2-3 weeks. However, by the third week you are already socially acceptable again. Sport and lifting heavy objects should be avoided for approx. 6 weeks.
